There is another fmt parameter YouTube utilizes to test out 720p HD video (like the video files on iTunes) that rolled out earlier last month. It's &fmt=22 and in order to make this code available to your videos, try to upload videos with a resolution of 1280 x 720 in either .mp4 or .mov formats. (I also suggest encoding in AAC sound.)
So respectively:
&fmt=6 (HQ videos with mono sound.)
&fmt=18 (HQ videos with stereo sound.)
&fmt=22 (HD 720p videos with stereo sound.)
&fmt=22 will not work on videos that do not fulfill the 1280 x 720 requirement.
And also, be aware that &fmt=22 will entail much longer load times, and in some cases - videos will experience audio lag. Like before, quality will ultimately be determined by bitrate and proper encoding.

I am in no way affiliated with this group (nor have I been aware of their existence until yesterday), but apparently, there's a site called http://videohelp.com in which a particular group of people have been conducting their own investigation with YouTube's recent implementation. According to their forums, they were able to determine the code "&fmt=6" as well as "&fmt=18" a couple of weeks before I did, and despite all the nasty comments they've been sending me, my sincerest apologies goes out to them. Like I said, I didn't mean to steal anyone's glory.
The only difference is that I immediately shared the code with you guys (charlestrippy, among others) as soon as I was able to conceive the code via greasemonkey.
Again, I do apologize if you feel that I'm wrongfully accredited. Send me a message, and I'll try my best to give you a mention.
